Q: What is Medical Makeup?
Dermablend Medical Makeup is manufactured to deliver lightweight, consistent, and flawless skin coverage and is:

Cover Crème foundation - “Full coverage foundation for moderate to major skin flaws delivers 12 hours of consistent color wear. It is water-resistant and smudge-resistant when used with Setting Powder.” For moderate to major skin flaws such as Vitiligo, Birthmarks, Rosacea, Pigmentation Disorders, Post-operative bruising, Scars, and Burns.
Loose Setting Powder - “Micronized talcs that set make-up and reinforce wearability. Transparent upon application and fragrance free."
Leg & Body Cover Foundation - “Full coverage body foundation with a lightweight formula that easily glides on larger skin surfaces. Quick-Fix Concealer – “an improved lightweight formula with high purity pigments, Quick-Fix offers improved skin tone shades and texture with long lasting wear. For moderate to major localized skin flaws, Quick-Fix provides buildable coverage that wont enhance the appearance of wrinkles or pores. For skin flaws such as scars, bruises, freckles, dark under eye circles, blemishes, and age spots.”
Q: Do you provide samples of medical makeup?
No. Unfortunately, the manufacturer does not provide us with makeup samples to supply to our customers.
Q: If I buy makeup and then find out it’s the wrong shade, can I return it for a refund or exchange it for a different color?
We at 360-5 customer care understand the difficulty that customers may experience when choosing a color. Unfortunately, it is within our return policy to not accept any returns on items from the skincare line that have been opened or used. The policy states:
“…no item from the skin care line, including but not limited to soaps, lotions and crèmes may be returned for any reason, except for container damage during shipping or the order was processed in error by Cleveland Clinic Wellness Enterprises LLC.”
Q: Which stores carry medical makeup in my area?
Our Wellness Store, located in Cleveland, Ohio on Cleveland Clinic’s Main Campus, carries the Cover Crème foundation, QuickFix concealer, Loose Setting Powder, and Leg & Body Cover. For other dermablend products, or if you live outside the Cleveland area, call your nearest retail department store to inquire as to whether they carry the Dermablend skincare line.
Q: How do I know which makeup color is best for my skin tone?
Log onto our site and click on the picture of the product to get a larger view of the makeup color. Use the color palate provided to choose the shade that best matches your skin tone. Otherwise, you may try looking for the Dermablend skincare line at your major retail department store and see if they offer color consultations.
Placing an order
Q: How can I purchase products from your website?
We currently do not take orders over the phone. You will need to create an account on our secure website to order health and wellness products. To create an account:
1. At 360-5.com, click the Login/Register tab on the top of the page.
2. If you have an existing account, enter your e-mail address and password to "Sign In". If you do not yet have an account, enter your e-mail address and create a password (Password must include 1 Capital letter and 1 numeral) to "Create Account".
3. You have created your account and are ready to shop and start earning your wellness rewards.
4. When you have chosen a product, click "Add to Bag". Indicate desired quantity, and enter Promo/Rewards code it applicable. You may either return to the shop or click on "Secure Checkout". Enter your shipping information (view restrictions below) and click "Next". Enter your Billing Information and click "Next". In the Order Review, confirm that your order information is correct and click "Submit Order". Your order has been placed and will be delivered to you within 5-7 business days.

Q: Do you ship to Canada or Internationally?
No. We do not have access through customs to ship to Canada at this time. We are working to implement this in the near future. We also cannot ship to international addresses, P.O. boxes or APO/FPO addresses. We apologize for the inconvenience.
Q: How much does it cost to ship to Puerto Rico, Hawaii, or Alaska?
The shipping cost to these three locations is $16.95. Shipping within the US is $7.95. Receive Free Shipping on most orders over $40.
Q: Do you ship to P.O. Boxes?
No, our packages are delivered via UPS Ground, and therefore we cannot deliver to P.O. Boxes. We apologize for the inconvenience.

Q: Where is your Wellness Store located? What are the hours?
The store is located on the Cleveland Clinic Main Campus in the Sydell & Arnold Miller Family Pavilion (Building J) at 9500 Euclid Ave., Cleveland OH 44106.

Q: How do I create an account on your website?

6. You will receive an email confirming that your new account has been created
Q: My password doesn’t work and I can’t log into my account. What do I do?
There may be several reasons why you may have trouble logging into your account. Try the tips below, and if you’re still having trouble, contact Customer Care for help.
1. Check that your e-mail address is spelled correctly, and make sure that you input the same email address used when setting up your account.
2. Check that your password is spelled correctly. Your password is case sensitive. Remember that it must contain at least one capital letter, one lower case letter, and one numeral.
Q: When I try to create an account using my CCF Employee #, it says invalid ID. Why?
In order to register an account on 360-5 as a CCF employee, you must be an actual employee of CCF, not a volunteer, contractor, research fellow, student, etc. If you are not an employee or fall into one of these categories, return to the website and create an account as an external user. You will still be able to get points and rewards, just not those that are designated specifically for CCF employees, such as the flu shot points.
Wellness Rewards/Point Redemption
Q: What are Wellness Rewards? How do I earn points?
The 360-5 Wellness Rewards Program rewards customers for taking action and making decisions that help improve one’s health and wellbeing. Customers earn numerical points (Point) for purchasing products at 360-5 retail stores and at 360-5.com. By rewarding the purchase of wellness products, the 360-5 Wellness Rewards Program provides an incentive to its customers to make healthy choices as they travel on their journey towards wellness.
Q: How do I redeem my Wellness Rewards?
When you have achieved the minimum point balance you may print a “Rewards Coupon” to use either in store or online. Log into your 360-5 account and choose the amount you wish to redeem , based upon your number of points. Coupons are redeemed in amounts of $1, $5, $10 and $20.
Rewards Coupons (i) can be redeemed for merchandise only as offered by 360-5—either in store or online, (ii) must be used in their entirety at time of purchase and (iii) may not be redeemed for gift cards, store credit, select products, on-line services or service items. Reward Coupons have no cash value and cash will not be paid for unused portions of the Rewards Coupons. Merchandise purchased with a Rewards Coupon may be exchanged for 360-5 merchandise only. Rewards Coupons are transferable and 360-5 reserves the right to limit the time within which you may redeem Rewards Coupons.
*Note: 360-5 will not be responsible for lost or stolen Rewards Coupons. Points will not be earned on sales tax, reward redemptions, prior purchases, or returned merchandise. 360-5 may change the minimum point redemption threshold at any time without prior notification.
